Guinea Pig IV: Mermaid In A Manhole is a 1988 Japanese gore horror film directed by Hideshi Hino, and starring Shigeru Saiki, and Mari Somei. The film is the fourth installment in the Guinea Pig series.
After the death of his wife, an artist decides to live in solitude and dedicate his time to painting. One day, he decides to go down into the sewers where he finds a young beautiful mermaid. It seems that after the drying of a river she ended up stranded there. It looks like during her staying in the sewers, the mermaid developed some king of infection and her body is getting full of some sort of boils, and she won't stop bleeding.
